Billing Questions

If you have a question regarding a billing matter, please call 623-825-5200 and someone from our Billing Department will be happy to assist you. You may also email us at billingapg@gmail.com. Please allow at least 7 business days for a response.

Medical Records

If you would like your medical records from another practice transferred to our office, please download the Records Release Form and we will be happy to submit the request on your behalf.

If you would like a copy of your medical records from our practice sent to you or another physician, please download the Records Release Form and mail or fax it our office. Please allow up to 14 business days to prepare your records.
